Please read this contract carefully as it is a legally binding document. Initial all paragraphs and complete the last page. All fields are mandatory. If you cannot fill out a section or disagree with the contents of any portion, please contact Spay and Stay to discuss. Thank you for adopting! *All cats come with a home again microchip. See below for instructions on registering the microchip number: www.homeagain.com/membership-services.html to register your microchip number - microchips are unregistered at time of adoption. To register it, create your own account on Home Again’s website. This is a free service.

Terms & Conditions*
- I agree to keep this cat indoors due to the dangers of life-threatening parasites, viruses, injury and abduction that outdoor cats face.
- I agree to treat this cat appropriately, never putting the cat’s health or safety in jeopardy and provide fresh food, water, litter, toys and stimulus for it daily, as well as providing regular veterinary care and required vaccinations. (S&S considers declawing as jeopardizing the cat’s health/safety)
- I agree that Spay and Stay has discussed this cat’s medical history, behaviors, personality and habits with me.
- I agree to obtain all city licenses as required by my local government.
- If at any time I cannot continue to keep this cat for any reason, I will immediately contact Spay and Stay for options. I understand the adoption fee is non-refundable.
- I affirm that no members of my household, including myself, have been convicted of any animal welfare violations or animal cruelty.
- Spay and Stay may make a scheduled follow-up home visit at a 3/6/or 12 month interval after the adoption.
- I agree that I am the owner of this cat, and I will accept full responsibility for it at my own risk, and release Spay and Stay from any liability for any arising health issues, damages or expenses incurred with cat ownership.
- In the event this cat is lost or dies, or I move or change any of my information, I will notify Spay and Stay immediately.
- I understand that failure to perform these above actions will constitute a breach of contract therefore authorizing a representative from Spay and Stay to reclaim the cat.
- I agree that I was provided with copies of this cat’s medical records and rabies certificate.
- I understand that the undersigned "contract initiator" attests that all of the above information is true and correct to the best of their knowledge.
- I hereby attest that I have read all of the above disclosures and further understand that I am entitled to keep a signed copy of this contract.
